| name | Name of the insurance company. |
| cms_id | Centers for Medicare and Medicaid Services (CMS) identification number of the insurance company. |
| freeb_type | Code the identifies the payer type, such as Medicaid, self pay, or disability. |
| x12_receiver_id | Identification number for the EDI clearinghouse used by the insurance company. |
| insurance_type | Insurance type, such as contract, public, private, or group health coverage. |
| auto_adjustment | Indicates whether the user automatically posts contractual adjustments on encounter close. |
| adjustment_code | Identification number for an adjustment. |
