A Multidisciplinary Treatment Plan (MDTP) facilitates the involvement of multiple therapy disciplines in your behavioral health agency. Using plans enhances the interaction between different providers, counselors, or other specialists to provide patients with better treatment outcomes

This topic contains the following sections:

Create a Plan for a Patient

You create Multidisciplinary Treatment Plans for individual patients. Each plan is a structured workflow that measures and tracks the progress of treatments for a particular problem or diagnosis. 

  1. Click the Patients tab. 
  2. Click Patient Search/Add in the left hand navigation bar, and then search for the patient.
  3. Click Multidisciplinary Treatment Plan in the left hand navigation bar.
  4. Click Add Plan.
  5. Enter the plan header information.
    • Name for Plan: Select the plan name from the list.
    • Start Date: Select the start date for the plan.
    • End Date: Select the end date for the plan.
    • Date Last Reviewed: Enter the last review date if you are modifying an existing plan. Not applicable if you are creating a report. 
  6. Select the care providers who are required to sign off on the treatment plan from the Available Providers field.
  7. Optionally enter the recovery goals, if any.
  8. Click Add Condition, and then enter the required information in the Add Medical Problem page. Click Save & Exit when finished. The condition is added to the Condition List.
  9. Click Edit next to a condition to define the needs, objectives, and interventions required to treat the condition.

  10. Select a minimum of 3 strengths. 
  11. Select a minimum of 3 weaknesses. 
  12. Enter any cultural issues, preferences, or any other concerns for the patient.
  13. Select the recommended initial discharge plan.
  14. Select the criteria the patient must meet for discharge.
  15. Select the applicable client agreement.
  16. Select the checkbox to accept the agreement if the patient is covered under Medicaid Title XIX/XXI and/or Serious Mental Illness (SMI). 
  17. Enter the patient and guardian signatures, if applicable.
  18. Save the form and view the signature of the selected providers. The selected providers receive a reminder about the assigned plan.
  19. Click Save to save the plan.

Edit a Condition

Define the needs, objectives, and interventions required treat each condition included in the plan. 

You can edit a condition when you create a plan, or when you add progress updates to an existing plan. To edit a condition in an existing plan, search for the patient, and then click Multidisciplinary Treatment Plan in the left hand navigation bar.

  1. Click a condition. The condition appears in the Condition List.
  2. Click the Edit button next to a condition to add treatment objectives.
  3. Enter any identified needs. Click Guide to select a predefined need.
  4. Click the Add Objective button to the enter a list interventions and skill sets needed to achieve treatment goals. Click Guide to select a predefined objective.
  5. Click the Add button under Service Categories to specify service categories and providers. The application displays the service code that corresponds to the service category.
  6. Click the Add Measure button to specify initial and desired goals to measure progress against. Click Guide to select a predefined measure.
  7. Click Add Intervention to add interventions, which are activities performed as part of reaching the objective. Click Guide to select a predefined intervention.
  8. Click Save.

Add Progress Updates to an Existing Plan

You can update a Multidisciplinary Treatment Plan to track the patient's progress. 

When you add a progress update to a plan, the page displays the following information:

  • Form to Include in Progress:  A list of forms that are included in the plan. The form information appears in the Encounter Summary page when you view the encounter.
  • Identified Needs:  The objectives that are added when you created the treatment plan. 
  • Plan Overview:  Overview information about the plan that you entered when you created it.
  • Service Plan Rights Acknowledgment for Persons who are Title XIX/XXI and/or SMI:  The attestation information provided when you created the treatment plan.

When you add a progress update to a plan, you must first create a new encounter or select an existing encounter to associate with the plan.

  1. Click the Patients tab. 
  2. Click Patient Search/Add in the left hand navigation bar, and then search for the patient.
  3. Select the encounter to associate with the plan.
    • Click the New Encounter link next to the patient's name to create a new encounter.
    • Select an open encounter from the Encounter History menu.
  4. Click Multidisciplinary Treatment Plan in the left hand navigation bar. 
    The page displays a list of existing plans.
  5. Click the Add Progress button for the plan you want to update.
  6. Select the link for a form to include in the update under Form To Include in Progress. The selected form opens in a popup window.
  7. Fill in the form, and then click Save.
