Creating a Falls Risk Assessment Report
In this article, we will outline the process of creating a falls risk assessment report using the reporting tool.

  1. Begin by navigating to the reports tab and selecting "New Report." This will open a box for report creation.
  2. Choose the option to select all information to ensure comprehensive data inclusion.
  3. Define the focus of the report. In this case, we will look for falls risk assessments. Select the falls risk assessment option.
  4. Review the default fields included in the report. These typically include:
    1. Who edited the last falls risk assessment
    2. What actions were taken (created, old value, new value, edit date)
  5. Remove unnecessary fields. For this report, we do not need to know who edited it. Retain the creation date and edit date.
  6. Add relevant fields to the report:
    1. Include the person’s name as the first column.
    2. Add the date of the falls risk assessment to show when it was created.
    3. Consider including who created the assessment.
  7. Incorporate custom field questions that are relevant to the falls risk assessment. For example, include:
    1. History of seizures
    2. Self-injurious behavior
    3. Staff assistance required
    4. Scoring details
  8. If applicable, include additional fields such as:
    1. The RN who signed the assessment
    2. The signature date
  9. Save the report and choose a public folder for accessibility. Confirm that the report is saved and visible in the reports section.
  10. To run the report, select the saved report from the list.
  11. Utilize filters to refine the data:
    1. Use the date range filter to specify a range for the edit date.
    2. Options include selecting a specific date or setting a custom date range (e.g., from July 1 to the current date).
  12. Apply the filters and review the results. The report will display the total number of records, and the total score based on the applied filters.
