In this article, we will outline the process of reviewing the personal health records section of a person's chart. This section is crucial for ensuring that all relevant health information is accurately documented and accessible.

  1. Access the Personal Health Records Section
    Navigate to the right side of the person's chart to find the personal health records section.
  2. Review Demographics Information
    Check the demographics info sheet. This section will only reflect the information that has been entered. If no information has been entered, certain fields will be blank, such as the ISP effective date, hair and eye color, intake meeting date, ethnicity, or race.
  3. Examine the Health Passport
    The health passport will display information based on what has been entered in various areas of the chart. Missing details such as guardianship, hair color, eye color, and height will not be reflected here.
  4. Check the Healthcare Management Plan
    This section will display any management plans that are currently in place. It is important for Direct Support Professionals (DSPs), Qualified Intellectual Disability Professionals (QIDPs), or anyone involved in interventions to easily access and review the healthcare management plan from this tab.
  5. Generate the Annual Preventative Health Screening Report
    You can run a report based on the date and the record type (male or female) to generate the annual preventative health screening report.
