Read Only Fields

With the exception of Dynamic Image Draw, which can be altered by the Form user, Read Only fields display static text or images, which users of the Form cannot delete, alter or overwrite. The text or images are inserted into the Form when it is being built or edited in Form Builder. The text or images can only be deleted, altered or overwritten from within Form Builder, while in Edit mode.

Static Text Line

Automatically displays a line of text, inserted into the Form while it is being built or edited in Form Builder. The line of text can only be deleted, altered or overwritten from within Form Builder, while in Edit mode.

Static Large Text

Automatically displays a larger field of text, inserted into the Form while it is being built or edited in Form Builder. The field size can be adjusted during Form Builder editing by clicking and dragging on the lower right-hand corner of the field. The field of text can only be deleted, altered or overwritten from within Form Builder, while in Edit mode.

Static Rich Text Field

Automatically displays a Rich Text field, containing text (with formatting), inserted into the Form while it is being built or edited in Form Builder. The field of text (with formatting) can only be deleted, altered or overwritten from within Form Builder, while in Edit mode.

Static Image

Automatically displays an image, inserted into the Form while it is being built or edited in Form Builder. The image can only be deleted, altered or overwritten from within Form Builder, while in Edit mode.

The Static Image field is configured in Form Builder, Edit-mode. Notice the “Upload Image” button which has been highlighted in yellow. It allows the builder of the Form to upload an image to be displayed in the Static Image field when it is viewed by a user of the form.

Dynamic Image Draw

Displays a limited set of drawing tools (draw, erase, line width, line color, clear, undo, redo). Also displays, within the field, any Static Image that may have been inserted while the Dynamic Image Draw field was being edited in Form Builder. Allows user to create simple line drawings on the Form. If a Static Image has been placed within this field during editing in Form Builder, the Form user can draw on top of the image, but cannot otherwise delete or alter the Static Image.

Drawing tools are displayed to the Form user, allowing user to draw freely. If a static image has been uploaded to this field, user would see the image and be able to draw on top of it.

In Form Builder Edit-mode, a Static Image can be inserted, upon which the Form user may dynamically draw. The drawing tools are not displayed in Edit-mode; they only appear in Preview mode or during use of the Form.

Visit Tools

Visit Tools allow a user to document an Encounter with a patient. User can indicate Services provided, add Diagnoses to those currently on the patient’s active Problem List,

Visit Services

Automatically displays a set of large buttons representing different procedure or treatment codes (CPT codes, HCPCS codes, etc.), as configured in Administration. Allows user to indicate which of these procedures or treatments were provided in an Encounter with a patient. When user clicks on one of these buttons, it is dynamically added to a list that is displayed on the Form, just beneath the buttons.

If user wants to include a procedure or treatment that is not specifically indicated on any of the displayed buttons, an “Add Service” Button is provided which, when clicked, displays a drop-down field and a search field, enabling the user to search for and add any procedure or treatment code in the database.

In Form Builder Edit-mode, the Visit Services field is added to the Form by selecting from a list of available fields.

Client Diagnosis

Automatically displays the selected patient’s currently active Diagnoses (Problem List). Allows user to add Diagnoses by clicking the Add Issue button.

When user clicks on the Add Issue button, a popup is displayed in which the user can select and document the new Diagnosis.

After clicking Save & Exit or Save & Add Medications, the new Diagnosis is added to the patient’s list of current active Diagnoses (Problems).

If user clicks on any of the current Diagnoses, a popup is displayed which allows user to modify or delete the Diagnosis from the patient’s list. A warning will appear after the user clicks the Delete button, where user can confirm or cancel the deletion. Note: User may have to scroll down on their DrCloudEHR™ screen to see the popup.

In Form Builder Edit-mode, the Form is configured to include the Client Diagnosis field by selecting it from a list of available fields.

Client Allergies

Automatically displays the selected patient’s currently active Allergies. Allows user to add Allergies by clicking the Add Allergies button.

When user clicks on the Add Allergy button, a popup is displayed in which the user can select and document the new Allergy.

After selecting an Allergy from the list, or adding one by entering text in the Title field of the popup, and then clicking Save, the new Allergy is added to the patient’s list of current, active Allergies.

If user clicks on any of the current Allergies, a popup is displayed which allows user to modify or delete the Allergy from the patient’s list of current Allergies. A warning will appear after the user clicks the Delete button, where user can confirm or cancel the deletion. Note: User may have to scroll down on their DrCloudEHR™ screen to see the popups.

In Form Builder Edit-mode, the Form is configured to include the Client Allergies field by selecting it from a list of available fields.

Client Medications

Automatically displays the selected patient’s currently active Medications. Allows user to add Medications by clicking the Add Medication button.

When user clicks on the Add Medication button, a popup is displayed in which the user can select and document the new Medication.

After selecting a Medication from the list, or adding one by entering text in the Title field of the popup, and then clicking Save, the new Medication is added to the patient’s list of current, active Medications.

If user clicks on any of the current Medications, a popup is displayed which allows user to modify or delete the Medication from the patient’s list of current Medications. A warning will appear after the user clicks the Delete button, where user can confirm or cancel the deletion. Note: User may have to scroll down on their DrCloudEHR™ screen to see the popups.

In Form Builder Edit-mode, the Form is configured to include the Client Medications field by selecting it from a list of available fields.

Other

Displays, on a field within a Form, almost any type of information recorded in DrCloudEHR™. Depending on the type of information displayed, the Form user may or may not be able to edit the field.

Existing Data Column

Displays almost any type of information recorded in DrCloudEHR™. Depending on the type of information displayed, the Form user may or may not be able to enter or edit information that is displayed within the field. In the example below, user may do so.

The type of information to be displayed is configured from within Form Builder Edit-mode, as follows:

Signatures

Let’s return to where we begin creating or editing a Form.

Scroll down towards the bottom of that page. You will notice a Section that is automatically created: Signatures. You control the Signature Types required to complete a form by selecting the available checkboxes.

For those Signature Types that correspond to your organization’s Access Controlled List (ACL) of users (i.e. Clinician, Supervisor, Doctor), you control which group or groups of users are authorized to sign for the Signature Types you’ve selected. Simply click the desired checkboxes from those that appear.

Note that Clinician ACL Group members are automatically marked as being authorized to sign for Clinicians, just as Clinic Administration ACL Group members are automatically marked as being authorized to sign for Supervisors.

Users who are required to cosign a Form for closure are sequentially notified. For example, a Supervisor is notified to cosign a Form after the Clinician has signed the Form. The Form is only closed (meaning it can only be modified with an Addendum) when the Form has been signed by all required signatories.
