Single-Select

Allows user to select one item from among several items in a drop-down list. User clicks drop-down arrow, moves cursor over desired item, and then clicks that item to select it.

Radio Buttons

Allows user to select one item from among several items by clicking on a radio button next to the item they wish to select. When Form is first opened by user, no radio button is selected. After selecting a radio button, user can change their selection, but cannot return to having none of the buttons selected.

Multi-Select

Displays a list of items with checkboxes. Initially checkboxes are empty, indicating no items have been selected. Allows user to select any number or combination of items by clicking on the checkboxes. When a checkbox is clicked, a check appears in it, indicating that the item has been selected. User may deselect any item they’ve selected by clicking the checkbox again. User may also click the “All” button to quickly select all of the items, or click the “Clear” button to return to the initial state of no checkboxes selected.

Matrix (Grid of Lists)

Displays a table relating the items of two lists. Allows user to select or enter a positive or negative number within a box at each intersection. Alternatively, Radio Buttons can be substituted for boxes, allowing user to single select within columns, but multi-select within rows. Can keep track of all values entered in the boxes as well as row totals, column totals and grand total of all rows and/or columns. Totals thus obtained can be used for scoring purposes. User may also enter text, such as Yes or No, in the boxes. The answers can then be tallied or scored. Here is an example of a grid with checkboxes:

Scoring

Allows scoring of any combination of fields in a Form, similar to the scoring that occurs in a Matrix field. Adds the ability to search for particular words or phrases within text fields, incrementing or decrementing the “score” by the number of occurrences of designated words or phrases.

Scoring is a very powerful feature of Forms. You may want to explore Scoring in greater detail, either on your own or with the help of DrCloudEHR™ Training and Customer Support. The following section of this document, “Adding and Configuring Scoring in Form Builder” will get you started.

Adding and Configuring Scoring in Form Builder

When you’re ready to add Scoring to a Form which you’ve built (or to a Form which has been provided or imported), go to Form Builder, find the Form to which you want to add scoring in the list that appears, and then click the Edit button for that Form. Now, in Form Builder Edit-mode, add a Field within a Group (In this example, we’ll create a new Group and add the new Field to this new Group. We’ll name this Field “total”.) In the “Kind of Data that will be entered” drop-down box, select Scoring.

Within Scoring Options, you’ll see all Sections of the Form listed. To the left of each Section in the list, you’ll see a small triangle. When you click on a triangle, a list of the Groups in the Section, with a triangle to left of each Group. Clicking on a Group will display a list of the Fields in that Group.

Each Field in the list will have a checkbox next to it. Click the checkbox next to a Field which you’d like to include in Scoring; a set of options will appear, depending on the type of data for which that Field was intended. The options which appear allow you to configure Scoring for the selected Field.

Configuring Scoring for Text Fields

Scoring can be configured to keep track of particular words or phrases which the user of a Form enters in a Text Field (Text, Text Area, Rich Text, etc.). As shown below, in Scoring Options for a Text Field, a word or phrase is entered in the “text to search for in Previous Field”. Then a selection is made for the number of “points to be awarded for finding text”. In this example, the number 10 is selected, meaning that 10 points will be added to the score whenever the designated word or phrase has been entered by the Form user in the corresponding Text Field on the Form.

As shown below, in Form Builder Preview-mode, when the designated word or phrase (in this case, “word”) is entered in the Text Field (for which we’ve just configured Scoring) by a user of the Form, 10 points are added to the total score.

Configuring Scoring for Date and Time Fields

In the case of Date and Time Fields, you can configure Scoring to tabulate points when a Date or Time Field has been populated with a time or date that is before, after or at a specified time or date. Here is an example of such configuration.

In this example, every time the date recorded in the Scoring-configured Date field is after 05/10/2017, 10 points are added to the total score for the Form.

Configuring Scoring for Number Fields

Configuring Scoring for Number fields is similar to configuring Scoring for Date and Time fields. The only difference is, instead of selecting a condition of before, after or at a certain date or time, the Form builder selects the condition of greater than, less than, or equal to the number recorded in the Scoring-configured Number field.

Configuring Scoring for Radio Button, Single-Select and Multi-Select Fields

For Radio Button, Single-Select and Multi-Select Fields, Scoring is configured to add points to the total score when selections are made by the Form user in the respective Scoring-configured selection fields.

In the following Radio Button example, the field is configured to add 1 point when the Form user selects “Occasionally”, 2 points when the Form user selects “Weekly”, etc.

As the Form user completes the Form, the points accumulate and are added to the total score for the Form.

Form builder list fields gives an ability to use the existing list of items that were created in the application.

A user can link the source to the list field by selecting a list from the list of topics at the settings ( "List external Topics")

Radio buttons can be used for single select items, in lace of boxes. in columns, but multiple choices or selections in rows. One can keep track of all values entered into boxes, and both row and column totals. There is the option for grand totals to be displayed as well.

Scoring fields allows the user to set score to the fields, when the fields are selected or enter values in form, system sums-up the score of the selected fields and displays the total score.
