Use the Activities of Daily Living (ADL) feature to document care assistance delivered by staff. You can track ad-hoc activities not outlined in a plan, or you can track performance according to the goals, objectives, and interventions outlined in the Multidisciplinary Treatment Plan (MDTP) created for a patient. The ADL feature ensures that individual records are maintained in a timely and accurate manner so that data can be used to analyze care improvement, an absolute requirement for achieving best levels of Value Based Reimbursement.

You can track a patient's daily activities on an on-demand basis, or you can track activities defined in a Multidisciplinary Treatment Plan.

View Activity History

If activity is defined in a Multidisciplinary Treatment Plan, click a text link in the Need column to open the plan, and then click on the condition to view needs and objectives defined in the plan.

  1. Click the ADL tab in the top navigation bar.
  2. Start typing the patient's name, and then select the patient in the results.
  3. Click Log/View Activity to view or update activity records. 
  4. Enter dates, then click Search. Current activities load in the History of Activities table for the patient below.
  5. Hover over the View link in the Notes column to view notes added for the activity.

Track Activities Defined in a Multidisciplinary Treatment Plan

  1. Click the Measures Image Addedicon. A table with a list of activities defined in the plan displays.
  2. Select the checkbox for an activity, and then add your notes.
  3. Select the icon that indicates the patient's performance.
    • Click the Self-Performance Image Added icon if patient performed task on their own.
    • Click the Support Image Added icon if the patient needed assistance from staff to complete activity.
  4. A new set of icons loads below based on your selection. Click the icon that best matches the patient's performance.
  5. Add your notes.
  6. Click Save. The activity is added to the History of Activities table for the patient.

Add Performance Notes for an Activity On-Demand

  1. Click an activity icon. Mouse over each icon for a text description.
  2. Select the icon that indicates the patient's performance.
    • Click the Self-Performance Image Added icon if patient performed task on their own.
    • Click the Support Image Added icon if the patient needed assistance from staff to complete activity.
  3. A new set of icons loads below based on your selection. Click the icon that best matches the patient's performance.
  4. Add your notes.
  5. Click Save. The activity is added to the History of Activities table for the patient.
