  • Sensory Perception: Assesses the individual's ability to respond meaningfully to pressure-related discomfort.
  • Moisture: Evaluates the degree to which the individual's skin is exposed to moisture.
  • Activity: Records the individual's level of physical activity.
  • Mobility: Assesses the individual's ability to change and control body position.
  • Nutrition: Evaluates the individual's nutritional status.
  • Friction and Shear: Assesses the degree of friction and shear the individual's skin is exposed to

Each category is scored, and the total score helps determine the level of risk. The Braden Scale is an essential tool for preventing pressure ulcers and implementing appropriate preventative measures.

To use the Braden Scale Form, follow these steps:

  1. Log In and Select Person
    • Log in to the system.
    • Select the person receiving services.
  2. Navigate to Forms Section
    • Find and select "Assessment."
    • Select Braden
  3. Locate and select the "Braden Scale Pressure" form.
  4. Fill Out the Form:
    • Complete all fields, which may include:
      1. Sensory Perception
      2. Moisture
      3. Activity
      4. Mobility
      5. Nutrition
      6. Friction and Shear
  1. Save the Form:
    • Save the completed form.
    • A record will be created with an assigned ID.
  2. Click on the Record
    • Ensure accurate entry of all relevant information, including details related to

  3. Review and Print
    • Click on the link on top-right “Print Braden Scale”
