Therapeutic Recreation Assessment

To use the Therapeutic Recreation Assessment Form, follow these steps:

• Log in to the system.
• Select the person receiving services.

• Navigate to the forms section.
• Find and select "Assessment".

• Locate and select the "Therapeutic Recreation Assessment" form.

• Complete all fields, including description, historical background, recreational involvement, data evaluation, and review.

• Save the completed form.
• A record will be created with an assigned ID.

• Ensure accurate entry of all relevant information, including personal history, vital lab results, medications, personal team and encounters, prescriptions, goals, and progress.

• Review the entered information.
• Print the form if needed, displaying goals, related objectives, and teaching strategies.

Braden Scale

To use the Braden Scale Form, follow these steps:

• Log in to the system.
• Select the person receiving services.

• Find and select "Assessment."
• Select Braden

• Save the completed form.
• A record will be created with an assigned ID.

• Ensure accurate entry of all relevant information, including details related to

Residential Court Report

To use the Residential Court Report form, follow these steps:

• Log in to the system.
• Select the person receiving services.

• Find and select "Reports."
• Select "Residential Court Report."

• Locate and select the "Residential Court Report" form.

• Save the completed form.
• A record will be created with an assigned ID.

• Ensure accurate entry of all relevant information, including details related to the court report.
• Make any changes if needed.

• Click on the link on the top-right labeled “Print Residential Court Report.”
