  • Nutrition: Evaluates the individual's nutritional status.
  • Friction and Shear: Assesses the degree of friction and shear the individual's skin is exposed to

Each category is scored, and the total score helps determine the level of risk. The Braden Scale is an essential tool for preventing pressure ulcers and implementing appropriate preventative measures.

These assessments and reports ensure that individuals receive comprehensive and personalized care, addressing both their recreational and medical needs as well as managing their legal affairs effectively.

To use the Braden Scale Form, follow these steps:

Log In and Select Person
  • Log in to the system.
  • Select the person receiving services.
Navigate to Forms Section
  • Find and select "Assessment."
  • Select Braden
Locate and select the "Braden Scale Pressure" form.Fill Out the Form:Complete all fields, which may include:Sensory PerceptionMoistureActivityMobilityNutritionFriction and ShearSave the Form:
  • Save the completed form.
  • A record will be created with an assigned ID.
Click on the Record
  • Ensure accurate entry of all relevant information, including details related to
  • Make any changes if needed.
Review and Print
  • Click on the link on top-right “Print Braden Scale”

This article will provide an overview of the main sections of DrCloudEHR IDD, detailing how caregivers can utilize these features to improve the quality of care they provide. 

Homepage

The homepage is the entry point for users and consists of four main components:

  1. Today's Agenda: A table showing the agenda for the day.
  2. List of Houses: A list of houses managed by the agency.
  3. Today's Tasks: A list of tasks that the logged-in user needs to tackle.
  4. Recently Viewed Persons: A small list of recently viewed individuals.

Persons Section

The Persons tab provides access to a list of individuals already added to the system along with their basic details. Key features include:

  1. Adding a New Person: Click the "New" button to open a form that replicates the intake form. Fill in the required details and click "Save" to create a new record.
  2. Editing Details: Click the pencil/edit icon next to a person's details to edit their information.
  3. In-House Admission: Navigate to the person's page and select "New Room Booking" to add details such as check-in date and time, and reserve a bed for the individual.

Forms Section

Within the Persons page, caregivers can access a variety of forms, including:

  1. Nutrition Assessments
  2. Speech Assessments
  3. Healthcare Management System
  4. Progress Notes
  5. Psychotropic Medication Review
  6. Behavioral Support Quarterly Forms (BSP Quarterly Report)

For example, to fill out the BSP Quarterly Report:

  1. Open the form from the list.
  2. Add the required details.
  3. The form gets populated and added to a table for tracking.
  4. Click "Print BSP Quarterly Report" to generate a PDF, which can be downloaded or printed.

Agencies and Facilities

The Agencies and Facilities sections provide lists of agencies and facilities along with their details. Users can:

 

  1. View Agencies: Click on an agency to see detailed information.
  2. View Facilities: Select a facility to view its details.

Calendar

The Calendar section allows caregivers to manage appointments and schedules:

 

  1. Viewing the Calendar: See the calendar for the day, including all assisting activities.
  2. Adding a New Appointment: Click "New Event" to log appointments, specifying whether they are clinical or service-based.
  3. Viewing Other Caregivers' Calendars: Access calendars of other caregivers to keep track of their schedules.

Reports and Analytics

DrCloudEHR IDD provides robust reporting and analytics tools to gain insights into daily routines and health trends. This feature helps in making data-driven decisions to enhance care quality.
